Middle School Athletics & Activities

All middle school athletic and activities programs are open to grades 6 – 8. Some teams and clubs may have participation limits in place due to coach/advisor/participant ratios.

How to Register

Fill Out Sports Physical Form

For Athletics: Sports Physical Form. (We will accept the 24-25 Sports Physical Form and the Clinic versions) Check the parent portal for valid physical date. Sports physicals are good for 3 years with a doctor’s signature. If there is not one listed on parent portal, please turn a paper copy into your child’s school before registration online. In addition to your child's clinic, other area offices offer sports physicals, a few examples are: area Minute Clinics, and Lake Marion Chiropractic Center . Once completed, sports physicals are valid for 3 years from the date of the exam. Sports physicals must be on file at your child's school to register for school sports. 

TO REGISTER FOR ATHLETICS/CLUBS :

  1. Create an Arbiter Registration account (only one account for your entire family - multiple households should choose one parent/guardian to be responsible for registration). 
  2. Activate your Arbiter Registrations account using the activation link that will be automatically sent to your emails. Note: this email may go into your junk folder). If you are having trouble creating your account, receiving the activation email, or logging in, you may contact support at registration@arbitersports.com
  3. Complete your registration by selecting the athletic or the activities and clubs registration program from the Lakeville Middle Schools Registration Site. You may need to put Lakeville Middle Schools and Minnesota in the search bar. You will need to know your student's ID # to complete registration.  Once you have completed your registration, you will receive an email notification confirming we have received your registration. 
  4. Registrations will be approved by the activity department once sports physicals are verified, registrations with no sports physicals on file at school will not be approved. Clubs do not need sports physicals.

Wrestling and Boys Swim start date is November 24, 2025. 

Important Notes for School Sports:

  • Sports Physical Requirements: All athletes must have a valid sports physical on file to be eligible for school sports. Your registration will not be considered if you do not have a valid physical.
  • If you have any questions or need to submit your sports physical, please email: rebecca.donner@isd194.org (North) or heather.allen@isd194.org (South)  

  • Well child physicals DO NOT count as a Sports Physical, however you can request that the clinic fill out a sports physical form based on this exam.
