ECFE Registration

  • Mother and her daughter Required Forms:

District Enrollment and Immunizations forms are required to complete ECFE registration. These forms should be turned into your class parent educator by your first class session.

Option A:  Download and email or print and return

If you have questions about these forms, please contact your class parent educator, email us at If you are unable to print these forms, copies are available at your class.

    Option B: New for 2021!

If your child is three years of age and has not had their Early Childhood Screening, you may register for both ECFE and Early Childhood Screening using the same forms!  Save time by filling out the Lakeville Screening Request and registering using the online registration link. This will fulfill both the ECFE and Early Childhood enrollment forms requirement and you do not need to complete any of the forms listed in Option A above.

Every attempt will be made to scheudule a screening onsite that corresponds with your ECFE class at some point during the term of your class.

*Please note that there are places to upload documents during online registration but it is not required to complete the online process.
