Required Annual Household Check-In
Annual Household Check-In
Each year, Lakeville Area Schools families are required to review, confirm and update their household and student information for back-to-school. Families will be asked to verify contact information, communication preferences and student health information.
You will not be able to review your student's teacher or class schedule until the check-in is completed.
We appreciate your support in updating this critical information so that we can best serve you and your students.
The household check-in for the 2024-25 school year is open now.
How to do the Check-In:
Step 1: Log into your Infinite Campus Parent Portal account
Step 2: Select 'MORE' from the menu along the left side of the screen
Step 3: Select 'Enrollment and Check-In'
Step 4: Select 'Start' next to 'K-12 Enrollment for Next School Year (2024-25) - Annual Household Check In'
Step 5: Select 'Begin Registration'
Important information to know before you get started:
- Only one parent/guardian per household will need to complete the Annual Household Check-In process.
- The Annual Household Check-In link is only active for one household per child. If your child has more than one household, the link will be active for the parent/guardian that lives in the household that is currently indicated as the child’s primary household.

Required Student Immunization Records
Student Immunizations for 2024-25 School Year
Minnesota law requires all children enrolled in school to be immunized against certain diseases from birth to 18 years of age, or to file a legal medical or conscientious exemption (Statute 121A.15, Rule 4604.0400). In Lakeville Area Schools, each student must have documentation that they have received certain immunizations or documentation of an exemption submitted before the first day of school (see policy webpage).
As we approach the beginning of the school 2024-25 school year, now is a good time to schedule an updated physical for your child and to check if your child’s immunizations are up to date. This chart shows what vaccines are required by the State of Minnesota by age/grade: Are Your Kids Ready? and When to Get Vaccines, Birth to 18 Years. This information is especially important for students entering kindergarten, 7th grade, 12th grade, and newly enrolled students.
If your child’s immunizations are out of date, you can use this form to document vaccines as well as medical or non-medical exemptions and return it to the school nurse before the start of the 2024-25 school year.

Attendance Reporting - Submit Absence Requests through Infinite Campus Parent Portal
Attendance Matters
Being in school, on time, every day is critical for your student’s success in school. With just two missed days each month, students can easily fall behind and have less academic success over the course of the school year and their academic career. Absences can also impact entire classrooms if teachers have to slow down learning to help students catch up.
It is recommended that students miss no more than nine days of school each year to stay engaged, successful and on track to graduation. Families can help their students succeed at all grade levels by building the habit of good attendance early in elementary school and paying attention to their attendance in middle and high school to keep them on track.
NEW! Submit Absence Requests through Infinite Campus Parent Portal
NEW for the 2024-25 school year, parents/guardians can submit absence, late arrival and leave early requests directly through the Infinite Campus Parent Portal. This can be done using your desktop computer or the Parent Portal app on your mobile device. This convenient new process is designed to streamline the attendance reporting process and minimize any attendance errors.

How Parents/Guardians will be Notified About Their Student's Attendance:
1. Any parent/guardian whose student is marked absent will receive an email, text message and phone call from their school. This first notification will come at 10:30 a.m. from elementary schools and at 10 a.m. from secondary schools.
- Parents/guardians may still receive this call even if they have excused this absence. This first notification is in the interest of student safety and alerts families/guardians of absences they may not be aware of. It is sent shortly after attendance is taken and does not always allow time for our staff to go through all the emails and phone calls we have received before the first call goes out.
2. A second notification will be made via email, text message and phone call to any parent/guardian whose student's absence is still marked unexcused. This second notification will come at 5 p.m. from elementary schools and at 3 p.m. from secondary schools.
Parents/guardians will have five days to resolve a marked absence. After the fifth day, it will be marked as an unexcused absence. Please refer to District Policy 503 on Student Attendance to see what is considered an excused versus unexcused absence.
Thank you for your support in ensuring your student is in school, on time, every day. Have a great school year!
