Annual Household Check In
Annual Household Check In
-
Each year, Lakeville Area Schools families are required to review, confirm and update their household and student information for back-to-school.
Families will be asked to verify contact information, communication preferences, and student health information. Families will also be asked to review and agree to the Acceptable Use Policy for student devices, and their school’s student handbook.
The household check-in for the 2023-24 school year is open now.
How to do the Check-In
Step 1: Log into your Infinite Campus Parent Portal account
Click here to learn how to sign into your parent portal
Step 2: Select 'MORE' from the menu along the left side of the screen
Step 3: Select 'Enrollment and Check In'
Step 4: Select 'Start' next to 'K-12 Enrollment for Next School Year (2023-24) - Annual Household Check In'
Step 5: Select 'Begin Registration'
Important information to know before you get started:
- Only one parent/guardian per household will need to complete the Annual Household Check-In process.
- The Annual Household Check-In link is only active for one household per child. If your child has more than one household, the link will be active for the parent/guardian that lives in the household that is currently indicated as the child’s primary household.
We appreciate your support in updating this critical information so that we can best serve you and your students.

Required Student Immunization Records
Minnesota law requires all children enrolled in school to be immunized against certain diseases from birth to 18 years of age, or to file a legal medical or conscientious exemption (Statute 121A.15, Rule 4604.0400). In Lakeville Area Schools, each student must have documentation that they have received certain immunizations or documentation of an exemption submitted before the first day of school (Policy 530).
As summer approaches, now is a good time to schedule an updated physical for your child and to check if your child’s immunizations are up to date. This chart shows what vaccines are required by the State of Minnesota by age/grade: Are Your Kids Ready? and When to Get Vaccines, Birth to 18 Years. This information is especially important for students entering Kindergarten, 7th grade, 12th grade, and newly enrolling students.
If your child’s immunizations are out of date, you can use thisform to document vaccines as well as medical or nonmedical exemptions and return it to the school nurse before the start of the 2023-24 school year.

The Minnesota Free School Meal Program will begin next school year (July 1, 2023). This program will provide free breakfast and lunch for students in Minnesota on school days. A la carte items will still be available to students for a charge, just like they are now.
Families who qualify for Educational Benefits should still plan to fill out an application for next school year, beginning July 1, 2023. This ensures that you will still receive discounts on other programs/fees and ensures the district doesn’t lose critical funding for students.
The Titan Family Portal is now LINQ Connect and all of your information has already been transferred over for you. Simply log in using your existing username and password. All your student, payment information, recurring payments, notifications and anything else you’ve previously set up will remain.

New Device Protection Plan
Annual Cost: $25 per student. Reduced price options are available based on Educational Benefit Status.
Coverage begins upon receipt of the payment. Payment must be submitted via the Infinite Campus School Store by October 1, to have coverage for the school year. New families joining the district will have 2 weeks from enrollment to add coverage.
The protection plan does not cover lost devices and lost, stolen, damaged or inoperable charging cords.
Please review the details of the device protection plan and payment information on the Lakeville Area Schools website.

Attendance Matters
Being in school, on time, every day is critical for your student’s success in school. With just two missed days each month, students can easily fall behind and have less academic success over the course of the school year and their academic career. Absences can also impact entire classrooms if teachers have to slow down learning to help students catch up.
It is recommended that students miss no more than nine days of school each year to stay engaged, successful and on track to graduation. Families can help their students succeed at all grade levels by building the habit of good attendance early in elementary school and paying attention to their attendance in middle and high school to keep them on track.
